Terms and Conditions for the 2025 CAQH Index Dashboard

Overview

These Terms and Conditions govern access to and use of the 2025 CAQH Index Dashboard, including the free public and paid subscription-based Index Pro version. By creating an account or purchasing a subscription, users agree to these Terms.

Free Version

  • Access and Features
    • On-screen text content from the CAQH Index Dashboard
    • Ability to download two PDF reports from 2025 Index: Executive Report and Methodology (unlimited downloads)
    • Access to archived CAQH Index reports
    • Ability to create a personal account to save access and preferences
  • User Information Requirements
    To create a Basic Access account, users must provide:
    • Name, organization, title, email address, stakeholder group, and any other required fields.
    • Consent to CAQH’s Privacy Policy.
  • Marketing and Sponsorship Sharing
    • By default, user information may be shared with CAQH Index Thought Partners.
    • Users may opt-out at the point of download or within their profile settings.
  • Usage Limitations
    • Free users may not reproduce, distribute, scrape, or sell any dashboard content, including downloaded PDFs.
    • On-screen dashboard content is for view-only use and may not be copied or exported.
  • Archiving and Access Expiration
    • Free users will retain access to archived Index Executive Reports and Methodology reports, subject to maintaining an active account and agreeing to updated Terms.
    • CAQH reserves the right to modify or remove content at any time.

Subscription includes:

  • 2025 CAQH Index Interactive Chartbook;
  • 2025 ROI Calculator;
  • 1 hour of CAQH Advisory Services; and
  • Access to the “Ask the Index” tool.

Subscription Fee: $899 per individual user license

Subscription Period: Begins upon payment and expires upon the release of the 2026 Index currently scheduled for early 2027.

Terms and Conditions

By purchasing an Index Pro Subscription (“Subscription”), subscribers (“Subscribers”) agree to the terms and conditions below (“Terms and Conditions”).

  1. Subscription Feature Details
    1. 2025 CAQH Index Interactive Chartbook (“2025 Chartbook”). Subscribers will have access to the 2025 Chartbook as long as Subscriber maintains a Subscription. The 2025 Chartbook is accessible by view only through a dashboard (not downloadable). The 2025 Chartbook will be archived and made available in a read-only format once the 2026 Index is released in Q1 2027.
    2. 2025 ROI Calculator (“ROI Calculator”). Subscribers will have access to the ROI Calculator through the CAQH website. Access to the ROI Calculator expires when the 2026 Index is released in Q1 2027.
    3. CAQH Advisory Services (“Advisory Services”). Subscribers will have access to one (1) hour of Advisory Services. Advisory Services entails assisting Subscribers with inquiries about the 2025 Index insights and other topics deemed related by CAQH and the Subscribers. The one hour of services must be used before the release of the 2026 Index in Q1 2027. Additional advisory hours may be purchased separately at any time.
    4. Ask the Index Tool. Subscribers will have access to the “Ask the Index” tool (LLM support).
  2. Subscription License. The Subscription license is for one individual only. No sharing or group access to the Subscription is permitted.
  3. Terms of Service. The Subscription and its benefits are subject to the Terms of Service on the CAQH website, which include but are not limited to the following:
    1. User Accounts restrictions (Section 3);
    2. Ownership of CAQH Content (Section 5), which includes the 2025 Chartbook, ROI Calculator, and Ask the Index Tool;
    3. Limitations on copying, sharing, modifying, and exploiting CAQH Content (Section 6); and
    4. Disclaimer of Warranties; Limitation of Liability (Section 13).
  4. Violations. If a Subscriber violates these Terms and Conditions, including any provisions in the CAQH website Terms of Service, CAQH has the right to immediately terminate the Subscription.
  5. No Refunds. No refunds will be issued if: (i) CAQH terminates the Subscription due to a Subscriber’s violation of these Terms and Conditions; or (ii) a Subscriber terminates this Subscription without cause.
